Job summary

EMPACT-Suicide Prevention Center, part of La Frontera Arizona, seeks a High Needs Case Manager to provide intensive, in-home and community-based behavioral health services to children and families in Arizona. The role emphasizes crisis intervention, family engagement, and multi-system coordination.

Responsibilities include lead meetings, document outcomes, and maintain a caseload of 15 families with a high level of intensity.

Qualifications

• Must meet BHT requirements with a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Behavioral health experience is required.
• Bi-lingual (Spanish) is a plus; valid driver license and fingerprint clearance are required.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention, transportation, stabilization, and parent training.
• Facilitate and document family, youth, and adult team meetings; engage natural supports and coordinate with partners.
• Maintain a caseload of 15 families with high-intensity service; ensure timely clinical documentation.
• Work a flexible schedule including evenings or weekends as needed; attend trainings and meetings.
• Coordinate with Clinical Coordinator/Director and external stakeholders to ensure continuity of care across systems.
• Develop and maintain relationships with other states, community mental health, and contracting organizations.
