EMPACT-Suicide Prevention Center, part of the La Frontera family, is a leading nonprofit delivering crisis intervention, behavioral health counseling and suicide prevention since 1987. We are an EEO/AA Employer dedicated to services for children and families.

We are looking for a Children's Case Manager at our San Tan Valley location. The role is full-time, with scheduling flexibility to meet client and family needs and to coordinate with partners for comprehensive care and referrals.

Qualifications

• Must meet BHT requirements with either a bachelor’s degree or higher and one year of behavioral health experience OR an associate’s degree and two years of behavioral health experience OR a high school diploma AND four years of behavioral health experience.
• Must have experience with the Child and Family Team process.
• Finger print clearance card or eligibility.
• Valid AZ driver license with clean record in past 3 years.
• Bi-lingual Spanish preferred.
• Bi-lingual Diff and Rural Diff hourly rates available.
• Staff must be at least 21 years old.

Responsibilities

• Provide direct service crisis intervention, support, transportation and education to families and children.
• Complete family needs assessments and referrals to partners and community services.
• Coordinate communications with other departments or collaborators to ensure continuity of care.

Skills

BHT requirements experience

Spanish (bilin gual) preferred

Education

Bachelor's degree or higher in behavioral health

Associate's degree in behavioral health

High school diploma + 4 years behavioral health experience
