Job summary

La Frontera EMPACT - Suicide Prevention Center in Tempe/Glendale seeks High Needs Case Managers to deliver direct crisis intervention, transportation, and family support services for children and families. The role manages about 15 families with a high level of intensity, coordinating with multidisciplinary teams to ensure continuity of care and timely documentation.

You will conduct home, school, and community visits, schedule and follow up on meetings, and link families to community resources

Qualifications

• Bachelor’s degree or higher plus 1 year behavioral health exp, or Associate’s with 2 years.
• Behavioral health experience is required.
• Experience with children is preferred.
• Fingerprint clearance or eligibility required.
• Valid AZ driver license and regular transportation.
• Must be at least 21 years old.

Responsibilities

• Provide direct in-home, in-office, in-school, and community services including crisis intervention and transportation.
• Coordinate family, youth, and adult team meetings; document and share outcomes with the team.
• Maintain and manage a caseload, ensuring timely and compliant documentation.
• Offer services aligned with Arizona Principles to support functional outcomes.
• Collaborate with clinical coordinators and other departments to ensure continuity of care.
• Develop relationships with state and community partners to coordinate services.
