Job summary

Cityblock is seeking a Pod Provider to deliver comprehensive primary and behavioral health care across multiple settings, including face-to-face and virtual visits. The role emphasizes chronic disease management, psychiatric medications management, and care coordination within an interdisciplinary team.

Requirements include MD/DO in Internal Medicine, Family Medicine, or Geriatrics, DEA licensure or willingness to obtain, and current US medical licensure.

Qualifications

• 3+ direct clinical care experience.
• Experience managing diverse patient populations with chronic conditions.
• US medical licensure and board-certified/eligible status.
• Basic Life Support certification required.

Responsibilities

• Conduct comprehensive clinical intake visits and follow-ups.
• Prescribe first and second line medications for depression, anxiety, and substance use disorders.
• Coordinate referrals to specialists per CBH guidance.
• Provide care planning, documentation, and chart reviews per policy.
• Participate in case conferences and panel management with the care team.
• Support development of clinical programs and care pathways.
