Cityblock Health seeks a Pod Provider to deliver longitudinal primary care across modalities, including face-to-face and virtual visits. You will manage chronic diseases, coordinate care with an interdisciplinary team, and support members throughout their care journey.

The role emphasizes clinical program development, documentation accuracy, and collaboration with care teams to meet member goals and improve health outcomes.

Qualifications

• Graduate from an accredited medical college in the United States or Canada.
• MD or DO in Internal Medicine, Family Medicine, or Geriatrics.
• DEA licensure or willingness to obtain.
• Certified in Basic Life Support for Healthcare Providers.
• Current valid license to practice medicine in the state of employment.

Responsibilities

• Conducts comprehensive intake visits for selected members.
• Follow-up after transition of care events or acute needs.
• Outreach to members requiring follow-up care.
• Provide clinical care per Cityblock Health protocols.
• Ensure accurate documentation and coding for encounters.
