Job summary

Cityblock Health seeks a Pod Provider physician to deliver comprehensive primary care across multiple modalities (Face to Face or virtual) in hub, community or home settings.

You will manage chronic diseases, support behavioral health, and work with an interdisciplinary team to develop member care plans and implement care pathways.

This full-time role offers a competitive salary in the $250,000–$265,000 range plus health, life and retirement benefits, equity participation, and PTO.

Qualifications

• 3+ direct clinical care experience.
• Experience managing diverse and complex patient populations.
• DEA license and current medical license in practicing state.

Responsibilities

• Conduct comprehensive clinical visits and follow-up per program guidelines.
• Prescribe psychiatric medications and manage chronic conditions.
• Participate in case conferences and care plan development with the team.
• Utilize EHR and scheduling platforms to document interactions and coordinate care.
• Provide leadership and supervision to Advanced Practice Clinicians.
