Qualifications

• 3+ years of clinical experience in primary care or chronic disease management.
• Med clearance requirements may apply and be evaluated.

Responsibilities

• Conduct comprehensive intake visits and provide longitudinal care focusing on chronic condition management.
• Proactively outreach to members needing follow-up care and ensure proper documentation with ICD-10/CPT codes.
• Collaborate with interdisciplinary team to develop and support member care plans and case conferences.
