Job summary

Cityblock Medical Practice, P.A. seeks a Pod Provider to deliver comprehensive primary care and chronic disease management across hub, community, and home settings. You will treat adults with complex conditions, collaborate with an interdisciplinary team, and guide clinical programs and care plans.

The role emphasizes preventive care, behavioral health integration, and equitable, person-centered care delivery. Expect supervision of APCs and participation in care conferences.

Qualifications

• 3+ years of direct clinical care experience.
• Experience managing diverse and complex patient populations.
• MD or DO with appropriate specialty; licensed to practice in the state.
• DEA licensure or willingness to obtain.
• Certified in Basic Life Support (BLS).

Responsibilities

• Conducts comprehensive clinical intake visits for selected members.
• Provides care for depression, anxiety, and substance use disorders.
• Collaborates with care team to develop and implement care plans.
• Maintains documentation and coding per policy.
• Mentors Advanced Practice Clinicians and supports supervision.
