Job summary

Cityblock Health is seeking a Board Certified/Eligible MD or DO in Internal Medicine, Family Medicine, or Geriatrics to lead primary care for complex populations in Michigan. The role focuses on chronic disease management, behavioral health integration, and preventive care within a team-based model.

You will work across face-to-face and virtual visits, coordinate with care teams, and ensure high-quality documentation, patient outcomes, and adherence to CBH protocols.

Qualifications

• 3+ direct clinical care experience.
• Experience with diverse and complex patient populations.
• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
• BLS certification for healthcare providers.
• DEA licensure or willingness to acquire.
• Current valid license to practice medicine in the state you are employed in with Cityblock.

Responsibilities

• Conducts comprehensive clinical intake visits and timely follow‑ups.
• Provides direct primary care and chronic disease management.
• Collaborates with interdisciplinary team to develop member care plans.
• Ensures proper chart documentation and coding (ICD/CPT).
• Oversees supervision of Advanced Practice Clinicians and supports care team.
