The role emphasizes chronic disease management, behavioral health integration, and social supports, with a focus on whole-person care across clinics in Columbus or Cincinnati and remote collaboration. Qualifications

• 3+ direct clinical care experience.
• Board-certified in a primary care specialty (Internal Medicine, Family Medicine, Geriatrics, or similar).
• Current US medical license in state of employment.

Responsibilities

• Lead clinical visits and care planning.
• Oversee care team and supervise Advanced Practice Clinicians.
• Coordinate with interdisciplinary team for member care plans.
• Ensure documentation meets CBH policies.
