Job summary

Cityblock Health in Ohio is seeking a Physician to lead clinical change and practice medicine within our value-based care framework. The role reports to the Vice President of Market Medical Practice and focuses on comprehensive, whole-person care that integrates physical health, behavioral health, and social support.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
• Current valid license to practice medicine in the state in which you are employed with Cityblock.
• Board certification in a major primary care specialty field.
• Certified in Basic Life Support (BLS).

Responsibilities

• Lead clinical visits and provide comprehensive care across physical, behavioral, and social health.
• Participate in case conferences and care planning with interdisciplinary team.
• manage panel of members, ensure quality gaps are addressed and documentation is complete.
• Supervise Advanced Practice Clinicians and provide clinical consultation as needed.
• Ensure compliance with CBH policies and chart reviews.
