Job summary

The Guidance Center seeks an Integrated Health Navigator to coordinate care across adult, adolescent, and child mental health services in partnership with community care providers. You will monitor care delivery, document progress, and intervene to keep plans on track while promoting patient empowerment and self-management.

You will work with PCPs, psychiatrists, therapists, and case managers to ensure high-quality, cost-effective health care, while meeting state criteria and maintaining

Qualifications

• Bachelor’s degree in a mental health field and 3 years working with minors and families.
• At least 1 year with adults with severe mental illness and 1 year with intellectual/developmental disability.
• Must meet MDHHS criteria for QMHP, QIDP and CMHP within 120 days of hire.
• Valid Michigan Driver License with good driving record.
• Ability to coordinate care with PCPs, therapists, and other providers; culturally competent.

Responsibilities

• Coordinate care for adults, adolescents and children using integrated treatment with community providers.
• Maintain awareness of services and supports available for consumers and families.
• Attend conferences, document care, review care plans and expedite tests/consults.
• Lead care coordination with health team to empower patients to manage chronic conditions.
• Provide self-management support and patient education.
