Qualifications

• Bachelor’s degree in social science or health-related field or 1+ years of experience caring for developmentally or intellectually disabled or behavioral health clients.
• Valid driver’s license and automobile insurance.
• Excellent written and verbal communication; ability to manage multiple priorities.

Responsibilities

• Provide care coordination to ensure specialty providers work together to improve member health.
• Develop and implement the Person-Centered Support Plan (PCSP) with member choices.
• Coordinate diagnostics, ambulatory care, and hospital services with providers.
• Guide members through social determinants of health, including access to exercise and healthy food.
• Participate in staff meetings, clinical conferences, and quality improvement initiatives.
