Job summary

Family Health Center – Southwest Indiana is seeking a Care Coordinator to support individuals by coordinating care across behavioral health, primary care, and community services. You will help patients understand their care plans, schedule follow-ups, and address barriers such as transportation and housing to improve health outcomes.

The role emphasizes collaboration with care teams, providers, and community partners, with a focus on safe transitions and ongoing engagement in care.

Qualifications

• 1–3 years’ experience in behavioral health, case management, or care coordination preferred.
• Experience serving high-risk or underserved populations preferred.

Responsibilities

• Coordinate care across systems and providers to connect behavioral health, primary care, and community resources.
• Assist individuals in understanding care plans, following up on appointments, and addressing barriers (transportation, housing, food access, benefits).
• Maintain timely, accurate documentation in the EHR and participate in interdisciplinary team meetings.
• Support safe transitions and continuity of care through communication with external providers and crisis teams when needed.
