Qualifications

Responsibilities

• Coordinates one-on-one primary care and behavioral health services for clients.
• Establishes relationships to support clients' wellbeing.
• Coordinates with healthcare providers to promote whole person care.
• Identifies and removes barriers to care (transportation, health literacy, etc.).
• Provides referral services addressing social determinants of health (SDOH).
• Supports preventive care and wellness screenings for various conditions.
• Develops health and wellness goals within integrated service plans.
• Communicates with care teams to ensure coordinated care.
• Transports clients as needed.
