NorthCare is hiring an outpatient care navigation professional to provide integrated health and behavioral health services for adults, children, and families. You will work within a multidisciplinary team to meet productivity standards while promoting recovery-oriented, trauma-informed care.

Responsibilities include assessments, care planning, linkage to services, and crisis management. The role requires ODMHSAS CM II criteria or equivalent licensure, a mix of education as listed, and valid

Qualifications

• Must meet ODMHSAS Case Management II criteria or equivalent licensure.
• Prefer at least 1 year of experience in a mental health or healthcare setting.
• Must have a valid Oklahoma driver’s license and reliable transportation.
• CPR, First Aid and Therapeutic Options certifications; or obtain within 30 days of employment.

Responsabilités

• Provides integrated services that are welcoming, engaging, and accessible, addressing physical and behavioral health needs.
• Promotes a warm, trauma-informed culture aligned with NorthCare values.
• Works with a multidisciplinary team to support health behavior change and coordinate care.
• Conducts assessments and develops care management plans integrating physical and behavioral health needs.
• Assists clients with eligibility for benefits, housing, transportation, education, employment, and follow-up care.
• Provides crisis management and safety planning, including collaboration with supervision as needed.
• Transports clients and conducts services in the community or at client homes.
