Job summary

The Behavioral Health Integration Coordinator- Adult or Child- will manage a dynamic case load and serve as a hub for the collaborative delivery within the larger behavioral health home team. The Coordinator will connect patients with community resources, supporting ongoing goals, and offer virtual, in-office, and in-home care delivery to address physical and mental health needs.

This role works with the care team to coordinate transitions, assist with Medicaid requirements, and document care

Qualifications

• Must be able to speak clearly, read, write, and comprehend the English language.
• Supports and works well in a team, able to work independently.
• Applies social work principles to daily interactions with clients.
• Able to work in fast-paced environments with interruptions.
• Excellent communication skills and problem-solving mindset.
• Experience with electronic health records; Excel is helpful.

Responsibilities

• Supports and promotes Covenant Health Ministry values.
• Acts as a hub coordinating care within the behavioral health home team.
• Links patients to community resources and helps remove access barriers.
• Coordinates appointments and may attend community appointments as needed.
• Educates patients on transitional supports and community resources.
• Collaborates with PCPs, Psychiatry, and other care team members.
• Assists patients with Medicaid requirements and renewals.
• Contacts community resources to establish availability and referrals.
• Documents individualized care plans and progress.
• Maintains compliance with HIPAA and MaineCare policies.
• Engages in program and team meetings and follows up after missed appointments.
• Performs other duties as consistent with the role.
