Centerstone is seeking an experienced behavioral health professional to provide client-centered support and therapeutic services in clients’ natural environments. The role emphasizes WRAP development, advocacy, and integrated care coordination to promote personal growth and health outcomes.

Responsibilities include ongoing case management, collaboration with families and providers, and ensuring services align with personal growth plans.

Formación

• Provide individual and group therapeutic interventions in client’s natural environment to facilitate growth.
• Assist with assessment and personal growth planning; link clients to services and advocate on their behalf.
• Monitor effectiveness of services and provide crisis services as needed.

Responsabilidades

• Develop Wellness Recovery Action Plans (WRAP) with clients and families.
• Provide outreach, advocacy, and follow-up per personal growth plan.
• Offer ongoing case management: assessment, planning, linking, monitoring, referral, advocacy.
• Coordinate with primary care and other care providers to support whole health.
• Assist clients with transportation to achieve wellness goals.
• Build natural support networks and assist with budgeting skills when indicated.
• Attend staff meetings and participate on clinical teams and quality assurance activities.

Bachelor’s degree or MH professional with 5 years experience
