Job summary

Centerstone is a leading nonprofit behavioral health system seeking a dedicated Case Manager to support individuals and families. You will conduct assessments, develop Individual Care Plans, and connect clients with resources, ensuring services meet program obligations and regulations.

You’ll collaborate with a team to monitor progress, advocate for clients, and integrate community supports. This full-time position offers growth in a mission-driven environment.

Qualifications

• Bachelor's degree required; Master's degree preferred.
• Four to six years of experience preferred.
• N/A certification/licensure.

Responsibilities

• Assesses treatment needs and develops patient care plans with the team to determine service levels.
• Completes assessments and develops/implements the Individual Service Plan and shares plans with client/family.
• Provides outreach, advocacy, support and follow-up and documents all outreach attempts.
• Assists clients in building a support team and links natural community supports to services.
• Develops relationships with local service and community resource agencies for client benefit.
• Provides transportation assistance to help clients achieve case management goals.
