Job summary

Centerstone is seeking a dedicated Case Manager to support clients across medical, social, and educational services. You will coordinate access, develop care plans, and advocate for continued entitlements, working closely with teams and community partners.

Responsibilities include direct client services, managing a life skills caseload, and ensuring effective communication among resources to maintain continuum of care. A bachelor’s degree and local 2 years' experience are preferred.

Qualifications

• Bachelor’s degree from a four-year college or university in psychology, social work, or related field.
• 2 years of experience preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct client services and case management.
• Coordinate access to medical, social, educational and other services.
• Develop and monitor person-centered care plans.
• Collaborate with team and outside agencies.
• Advocate and communicate with involved parties to ensure continuum of care.
• Complete client assessments and track progress.
