Job summary

Centerstone is seeking a dedicated case manager to provide person-centered assistance to adults and families, updating care plans and coordinating treatment across services to support recovery. You will act as primary contact for the client, maintain charts, and document progress in a timely manner within the CCBHC framework.

Qualifications

• Bachelor's degree preferred; experience equivalency may be considered in place of education requirement.
• Minimum of two years professional experience working with adults/youth and families in related settings.
• Certification in CANS/ANSA required.

Responsibilities

• Facilitates clients' use of family, social, and community resources.
• Maintain and update clinical records and care plans; communicate with managed care teams.
• Assist in admissions, discharges, and census management; ensure smooth flow of facility.
• Complete authorizations and assessments for treatment programs.
• Provide individual and group interventions in home, community, or office settings.
• Collaborate with criminal justice, child welfare, hospital, and educational systems.
