Job summary

Centerstone is seeking a dedicated behavioral health professional to provide person-centered care, update care plans, and coordinate treatment to improve client recovery. You will serve as the primary case holder and ensure documentation is complete for access to services across programs.

You will guide clients and families, communicate with care teams, and support clients in homes, communities, and clinics while using ASAM levels of care and CANS/ANSA tools as part of our CC BCH clinics model.

Qualifications

• Bachelor’s level preferred; experience may be considered in place of education.
• Two years of professional experience with adults or youth in behavioral health or related setting.

Responsibilities

• Provides person-centered assistance and updates care plans to support continuity of care.
• Serves as primary case holder and maintains client charts for access to services.
• Completes documentation timely and supports recovery with structured interventions.
• Interacts with family, community and other systems to coordinate care and resources.
• Explains insurance status and works with the client’s representatives and payees.
• Delivers individual and group interventions in home, community, or office settings.
