Job summary

Centerstone seeks a part-time clinician to support clients in their natural environment, delivering wellness-focused interventions and WRAP planning. The role includes assessment support, planning, linking to services and advocacy to help clients achieve personal health goals.

Responsibilities include coordinating with families and providers, monitoring service effectiveness, and assisting with crisis services as needed while maintaining quality and safety in care delivery.

Qualifications

• Education Level: Bachelor’s degree required or Mental Health Professionals (five years paid experience) with a high school diploma may be considered in place of education requirement upon approval.

Responsibilities

• Provide wraps wellness planning and consultation.
• Coordinate with clients, families and other providers to implement plans and follow-up.
• Deliver ongoing case management, assessment, planning, linking, monitoring and advocacy.
• Assist with linking clients to primary care, specialty care and community resources.

Education

Bachelor’s degree or Mental Health Professional experience
