Job summary

Health Connect America is seeking an Integrated Care Coordinator to deliver person-centered care, coordinate treatment plans, and support on-site clinic operations. The role emphasizes collaboration with nurse practitioners, care teams, and community resources to improve patient outcomes.

You will engage in assessments, care coordination, health promotion, and referrals while ensuring regulatory compliance and accurate documentation.

Qualifications

• Bachelor’s Degree in any discipline is required, with a preference for human services or related fields.
• Experience working with children and families in case management type/ community resource position.

Responsibilities

• Deliver comprehensive, person-centered care by planning, coordinating, and monitoring individualized treatment plans.
• Assist Nurse Practitioner with clinic appointment-related documentation and facilitation on site.
• Support marketing initiatives for new referrals and outreach to integrated care attributed members.
• Provide education on services and coordinate follow-up with care team.
• Coordinate care transitions, including hospitalizations and post-crisis debriefing when needed.
• Attend team meetings and act as liaison with professionals and agencies.
