You will develop Wellness Recovery Action Plans (WRAP), conduct wellness consultations, coordinate with primary and specialty care, connect clients

• Excellent written and oral communication abilities across diverse audiences.
• Experience in customer-facing service and care coordination.
• Familiarity with community resources and care planning.
