Job summary

Oak Street Health is seeking a Behavioral Health Specialist to deliver care management and evidence-based short-term psychotherapy to patients with behavioral health needs. The role supports Care Teams across regional clinics and may include remote work days based on program metrics.

The specialist will assess patient needs, develop care plans, and coordinate with psychiatric consultants to provide timely referrals and treatment.

Qualifications

• Master's degree in mental health counseling from an accredited school.
• Highest clinical level license (or local equivalent).
• Experience with screening for mental health/substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured interventions (e.g., MI, Behavioral Activation, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to refer patients to Oak Street Behavioral Health Program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including telehealth handoffs.
• Document encounters and counseling sessions in the EMR.
• Participate in regional efforts to advance the program.
