Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and evidence-based short-term psychotherapy for patients in our primary care clinics.

You will work with care teams and a psychiatric consultant to assess needs, develop care plans, and coordinate referrals while delivering targeted counseling. A remote work option may be available based on program metrics.

Qualifications

• Master's degree in mental health counseling or related field from an accredited program.
• Current license at the highest clinical level or local equivalent.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling using evidence-based treatments.
• Document encounters in the electronic medical record.
