Job summary

Oak Street Health seeks a Behavioral Health Specialist to provide care management and evidence-based short-term psychotherapy within primary care clinics. You will coordinate with Care Teams and a psychiatric consultant to assess needs, develop plans, and refer to internal/external resources.

The role may include remote work days based on program metrics; requires a Masters in a mental health field and an active license, with US work authorization. 40 hours weekly, full-time position.

Qualifications

• Master's Degree in the field of mental health counseling/healing arts required from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques.
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth partners.
• Maintain timely documentation of encounters in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
