Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy within our primary care clinics. You will assess patients, develop care plans and coordinate with the psychiatric consultant to connect patients with appropriate resources.

The role supports evidence-based treatments, may involve remote work days based on performance and local leadership decisions, and requires a Master’s degree in a mental health field and a current license.

Qualifications

• Master’s Degree in the field of mental health counseling/healing arts required from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for common mental health and/or substance use disorders.
• Experience with assessment and treatment planning for common mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral, etc.).
• Working knowledge of differential diagnoses of common mental health and/or substance use disorders.
• Preferred: Proficiency in PC skills.

Responsibilities

• Collaborate with primary care teams to identify appropriate patients for the Behavioral Health Program.
• Develop care plans and provide or facilitate referrals to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant, including warm handoffs for telehealth visits.
• Develop relationships with primary care teams and communicate changes in care plans.
• Document patient encounters and counseling sessions in the EMR.
• Participate in regional/organizational efforts to advance the Behavioral Health Program.
• Possibly covering multiple clinics depending on needs.
