Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term evidence-based psychotherapy to patients with behavioral health needs. This role supports Care Teams across Oak Street Health primary clinics and may offer remote work days based on program metrics.

Responsibilities include screening, assessment, care planning, and referrals to psychiatric consultants or external resources; delivering brief treatments; coordinating telehealth visits; and

Qualifications

• Master’s degree in mental health counseling, social work, or related field from an accredited program.
• Highest clinical license level (LCSW, LPC, LMFT, or local equivalent) required.
• Experience with screening for mental health or substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief, structured interventions (e.g., MI, CBT, etc.).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the behavioral health program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth; update care plans.
• Build relationships with care teams and communicate changes in plans.
• Document patient encounters in the EMR accurately.
• Participate in regional and organizational efforts to advance the program.
• Cover multiple clinics as needed.
• Perform other duties as assigned.
