Job summary

Oak Street Health is seeking a Behavioral Health Specialist to coordinate care management and provide short-term psychotherapy for patients with behavioral health needs. The role involves collaboration with Care Teams and a psychiatric consultant, with potential remote work based on program metrics.

The position emphasizes evidence-based treatments, care coordination, and documentation within the EMR, with opportunities to cover multiple clinics as the program expands.

Qualifications

• Master's degree in a mental health field from an accredited program.
• Current highest clinical license or local equivalent.
• Experience screening for mental health or substance use disorders.
• Experience with assessment and treatment planning.
• Familiar with brief structured interventions (e.g., MI, Behavioral Activation).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify appropriate patients for referral to the Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and provide or facilitate referrals to the psychiatric consultant and/or external resources.
• Provide short-term counseling and evidence-based treatments as indicated.
• Coordinate care with the psychiatric consultant, including warm handoffs for telehealth visits and collaboration on care plans.
• Develop relationships with primary care teams and communicate changes in behavioral health care plans.
• Accurate and timely documentation of patient encounters and counseling sessions in the EMR.
• Participate in regional and organizational efforts to advance the Behavioral Health Program through workshops and surveys.
• Possibly covering multiple clinics, depending on panel sizes.
