Job summary

Oak Street Health, part of CVS Health, is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy to patients with behavioral health needs. You will collaborate with Care Teams and a psychiatric consultant to coordinate care across multiple clinics.

The role includes screening, assessment, developing care plans, referrals, and delivering brief evidence-based therapies. Remote work days may be possible based on metrics and program performance.

Qualifications

• Master’s Degree in the field of mental health counseling/healing arts is required.
• Highest clinical level of license (or local equivalent) is required.
• Experience with screening for mental health and/or substance use disorders is required.
• Familiarity with brief, structured intervention techniques is preferred.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Oak Street Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with the psychiatric consultant, including warm handoffs for telehealth visits.
• Communicate changes in behavioral health care plans to primary care teams.
• Document patient encounters and counseling sessions in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
