Job summary

Oak Street Health is seeking a Behavioral Health Specialist to join our regional care teams. You will provide care management services and short-term evidence-based psychotherapy to patients with behavioral health needs, coordinating with primary care teams and a psychiatric consultant.

Qualifications

• Master's Degree in mental health counseling/healing arts required.
• Highest clinical license (or local equivalent) required.
• Experience with screening for mental health/substance use disorders.
• Experience with assessment and treatment planning for mental health/substance use disorders.
• Familiarity with brief structured interventions (e.g., MI, BA, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify patients for referral.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals with psychiatric consultants.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth.
• Build relationships with care teams and communicate plan changes.
• Document encounters and counseling in the EMR.
• Support regional efforts to advance the Behavioral Health Program.
• Possibly cover multiple clinics as needed.
• Handle other duties as assigned.
