Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs. You will work with Care Teams regionally, assess patients, make referrals, and deliver evidence-based treatments. There is potential for remote work days based on metrics.

You will collaborate with psychiatric consultants, develop care plans, and ensure timely documentation in the EMR while supporting clinics across the region.

Qualifications

• Master's Degree in the field of mental health counseling/healing arts from an accredited school.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and facilitate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs.
• Build relationships with primary care teams and communicate changes in care plans.
• Accurate documentation of patient encounters in the EMR.
• Participate in regional efforts to advance the Behavioral Health Program.
