Job summary

Suncrest Hospice is seeking an Intake Coordinator to support referrals, scheduling, and chart flow in our Walnut Creek, CA office. You will handle patient referrals, assist with admissions, and coordinate with the sales team and clinical staff to ensure timely care transitions.

The role requires strong phone etiquette, insurance experience, and proficiency with Word/Excel. Weekend/holiday coverage is required as needed in a growing national hospice program.

Qualifications

• Two-year college degree preferred or equivalent hospice/healthcare experience.
• Experience working with insurance.
• Proficient with Microsoft Word and Excel.
• Strong time management and organizational skills.
• Strong ability to multi task and prioritize.
• Excellent telephone and customer service skills.
• Proficient with medical terminology.

Responsibilities

• Answer telephone inquiries and channel them appropriately.
• Assist with tracking and processing referrals.
• Assist with audits and chart reviews as assigned.
• Upload documents to electronic medical records system.
• Provide feedback on process and flow improvement.
• Assist with scheduling goals of care and admission visits.
• Work with sales team to receive incoming referral information.
• Assist on weekends/holidays as needed.
• Perform other duties as assigned by Director of Intake.
