Health Link Home Health & Hospice in Walnut Creek, CA is seeking a self-driven Scheduling Intake Coordinator for an immediate on-site opening. You will manage and coordinate home health schedules, assist with intake, and ensure timely staffing while handling high call volumes with professionalism.

Ideal candidates have 1 year of home health scheduling experience, strong organizational and communication skills, and the ability to work independently in a fast-paced environment.

Qualifications

• Previous scheduling or intake experience (home health required).
• Detail oriented with excellent verbal, written, and interpersonal communication skills.
• Friendly, professional phone presence and comfortable handling high call volume.
• Strong organizational and time-management skills.
• Ability to prioritize and multitask in a fast-paced environment.
• Ability to work independently with minimal supervision.
• Willingness to work occasional weekends and holidays as required to support patient care and operational coverage.
• On-site at our Walnut Creek location.
• Full-time position; not remote.

Responsibilities

• Maintain and coordinate schedules for home health services
• Assist with intake and process incoming referral information
• Ensure cases are assigned and staffed within established timeframes
• Communicate schedule updates to clinicians, patients, supervisors, and referral sources
• Manage high call volumes with professionalism and a pleasant demeanor
• Respond to urgent or emergency scheduling needs
• Provide direction to field staff as needed to ensure appropriate patient coverage
• Report scheduling changes and client concerns to supervisors
• Verify time records against schedules
• Follow up on scheduling issues reported by on-call staff
• Attend required meetings and training
• Maintain compliance with agency policies and procedures
• Perform other duties as assigned
