Job summary

Brighton/Suncrest Hospice in Walnut Creek, CA is seeking an Intake Coordinator to support referrals, scheduling and communications with insurance and healthcare providers. You will work in a growing hospice environment focused on high-quality patient care and efficient intake processes.

Qualified candidates bring at least two years' hospice or healthcare experience, strong phone skills, and proficiency with Word and Excel.

Qualifications

• Minimum of two years' experience preferably in hospice or health care setting, or graduation from a one- or two-year college preferred.
• Experience working with insurance
• Proficient with Microsoft applications including Word and Excel
• Strong time management and organization skills
• Strong ability to multi task and prioritize
• Excellent telephone and customer service skills
• Proficient with medical terminology

Responsibilities

• Answers telephone inquiries and channels them appropriately
• Assists with tracking and processing referrals
• Assist with audits and chart reviews as assigned
• Uploads documents to electronic medical records system
• Provides feedback on process and flow improvement
• Assists with scheduling goals of care and admission visits
• Work with sales team to receive incoming referral information
• Assists on weekends/holidays as needed
• Other duties as assigned by Director of Intake
