Job summary

Health Link Home Health and Hospice is seeking a self-driven Scheduling Intake Coordinator for our Walnut Creek, on-site team. This full-time role focuses on maintaining and coordinating home health schedules and processing referrals to support patient care.

The ideal candidate has 1 year Home Health experience, strong communication, and the ability to manage high call volumes in a fast-paced environment. On-site requirement and immediate start in Walnut Creek.

Qualifications

• 1 year Home Health experience required.
• Strong communication and multitasking abilities.
• Detail-oriented with excellent organization.
• Ability to work on-site in Walnut Creek.

Responsibilities

• Maintain and coordinate schedules for home health services.
• Assist with intake and process incoming referral information.
• Ensure cases are assigned and staffed within established timeframes.
• Communicate schedule updates to clinicians, patients, supervisors, and referral sources.
• Manage high call volumes with professionalism and a pleasant demeanor.
• Respond to urgent or emergency scheduling needs.
• Provide direction to field staff as needed to ensure appropriate patient coverage.
• Report scheduling changes and client concerns to supervisors.
• Verify time records against schedules.
• Follow up on scheduling issues reported by on-call staff.
• Attend required meetings and training.
• Maintain compliance with agency policies and procedures.
• Perform other duties as assigned.
