Job summary

Responsibilities

• Receives and processes reports or requests for emergent services and verifies caller-provided information
• Collaborates with community partners to gather necessary information for eligibility
• Serves as initial contact to determine potential eligibility for social services programs and provides referrals
• Completes intake process for social services programs and coordinates documentation
• Works with program teams to facilitate intake and service provision
• Monitors the intake log for timely responses
• Maintains time accounting using appropriate systems and keeps case records compliant with guidelines
• Provides emergency on-call social services as assigned
• Contributes to staff orientation for intake procedures
• Participates in multidisciplinary and programmatic meetings with county staff and external partners
