Job summary

Transformation-House in Minnesota seeks an Admissions Coordinator to manage the intake flow for new clients. You will provide information about programs, coordinate admissions, and liaise with referring agencies.

You will document intake, schedule interviews, handle screening paperwork, and maintain wait lists while ensuring compliance with state licensing requirements.

Qualifications

• Minimum 1 year experience in a substance abuse disorder facility.
• 2-3 years related experience required, or equivalent combination of education and experience.
• Knowledge of Minnesota statutes Chapter 245G, HSLA, MOMA, VAA definitions.
• Willing to submit to and pass DHS Background Study.
• Basic computer skills and ability to use office equipment (copier, scanner, printer).

Responsibilities

Job description

This position will coordinate new client admissions, establish and maintain relations with direct prospects and referring entities.

Essential Duties and Responsibilities

• Provide general information about the company's programs and admissions process to direct prospects and third party referring agencies.
• Establish and maintain business relations with referring entities
• Schedule and perform intake interview
• Collect prospects documentation necessary for the intake screening
• Submit to the review team prospect client’s file for intake screening (approval for admission).
• Schedule new client date of admission
• Submit notification of admission to funder within 24 hours of new client date of illness
• Monitor and manage wait list
• Perform necessary data entry into EMR (Procentive)
• Determine client financial responsibility and monitor funder’s change (each first of the month)
• Monitor daily census log
• Maintain up to date and monitor the intake progression spread sheet
• Attend staff in services and meetings
• Other duties as assigned.

Minimum Qualifications Knowledge Skills and Abilities

• Minimum 1-year experience in a substance abuse disorder facility and
• 2-3 years related experience required, or
• Any combination of education and experience that provides equivalent knowledge, skills and abilities.
• Must know and understand the implication of Minnesota statutes Chapter 245G, and Minnesota Statutes, chapter 245A (Human Services Licensing Act-HSLA), sections 260E (Maltreatment of Minors Act-MOMA), 626.557 (Vulnerable Act-VAA), and 626.5572 (VAA definitions).
• Must be willing to submit to and pass DHS Background Study.
• Must have basic computer skills and ability to use office equipment such as copier, scanner, and printer.
