Job summary

Acap Me is seeking an Intake Specialist to help individuals and families access community resources. You will perform intake interviews, assess needs, coordinate services, and refer clients to appropriate programs, documenting information in our systems and ensuring eligibility requirements are met.

Qualifications

• Experience: At least two years of experience in a social service-based program.
• Skills: Excellent verbal and written communication, listening, customer service, and ability to work with diverse populations.
• Education: High school diploma or equivalent; associate degree preferred.

Responsibilities

• Gathers and records information from customers; assesses and prioritizes needs.
• Identifies services and coordinates referrals; assists with applications and follow-up.
• Performs data entry and maintains program records; schedules appointments.
