Job summary

Enhabit Home Health & Hospice is seeking an intake-focused professional to coordinate patient referrals and support transitions to services. You will work with branch and sales teams, applying regulatory guidelines to ensure compliant, timely care.

The ideal candidate has healthcare experience, strong customer service skills, and familiarity with Medicare guidelines; a high school diploma is required, and a nursing license is preferred.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
