Job summary

Enhabit Home Health & Hospice is seeking an intake/coordinator-style professional to coordinate patient referrals and serve as a resource for referral sources. You will collaborate with the branch and sales teams to ensure coordination and a smooth transition to services, while applying regulatory guidelines and governing processes.

The role requires a high school diploma and healthcare experience, with preference for nursing licensure in the state.

Qualifications

• Must possess a high school diploma or equivalent.
• At least two years of healthcare experience with one year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills, including operation of a mobile device.
