Job summary

Enhabit Home Health & Hospice is seeking professionals to coordinate patient referrals and support seamless transitions to services. You will work with branch and sales teams to ensure coordinated care while adhering to regulatory guidelines governing home health and hospice.

The role requires a high school diploma, healthcare experience, and strong customer service. Nursing licensure (RN/LPN) in the practicing state is preferred, with additional attention to intake and multi-location

Qualifications

• High school diploma or equivalent.
• At least two years in health care and one year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills, including operation of a mobile device.
• Preferred: RN or LPN license in state of practice.
• Experience with intake and multi-location environments is a plus.
