Job summary

Enhabit, Inc. is seeking a patient referrals coordinator to support the home health and hospice team. You will serve as a resource for referral sources and collaborate with branch and sales teams to coordinate transitions to services.

You will apply knowledge of Medicare guidelines and regulatory processes while delivering compassionate, compliant patient care and maintaining accurate documentation. A high school diploma is required; nursing licensure is preferred for advancement.

Qualifications

• Must have demonstrated customer service experience.
• Basic technology skills including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
