Job summary

Enhabit Home Health & Hospice is seeking an Intake Coordinator to coordinate patient referrals and collaborate with branches and sales teams for seamless transitions to services, while applying regulatory guidelines governing home health and hospice.

Ideal candidates have a high school diploma, 2+ years in health care and 1 year in home health or pediatrics, strong customer service, Medicare knowledge, and comfort with mobile devices.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
