Job summary

Enhabit Home Health & Hospice is seeking an intake/referral coordinator to coordinate patient referrals and serve as a resource for referral sources. You will work with branch and sales teams to ensure smooth transitions to services and apply Medicare and regulatory guidelines.

The role requires a high school diploma and a valid LPN/LVN license, with 2+ years in health care and 1 year in home health or pediatrics. Strong customer service and tech skills are essential.

Qualifications

• High school diploma or equivalent required.
• Valid LPN/LVN license required.
• 2+ years in health care with 1 year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health/hospice.
• Basic technology skills including mobile devices.
