Job summary

Enhabit Home Health & Hospice is seeking an Intake Coordinator to support patient referrals and coordination across branch and sales teams. You will serve as a resource for referral sources and ensure smooth transition to services.

Ideal candidates will have at least two years in healthcare, one year in home health or pediatrics, and a solid understanding of Medicare guidelines. A high school diploma is required, and comfort with mobile devices is expected.

Qualifications

• High school diploma or equivalent.
• At least two years in health care and one year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines governing home health or hospice.
• Basic technology skills including use of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure smooth coordination and transition to services.
