Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to support patient referrals and coordinate care across our branch network. You will work closely with sales teams and care staff to ensure seamless transitions to services while upholding Medicare guidelines.

The ideal candidate has at least two years in health care, experience in home health or pediatrics, strong customer service skills, and reliable technology use to manage mobile devices and documentation.

Qualifications

• High school diploma or equivalent is required.
• At least 2 years of experience in the health care industry and 1 year in home health or pediatrics.
• Demonstrated experience in customer service.
• Knowledge of Medicare guidelines that govern home health or hospice.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
