Job summary

ServiceSource, Inc. in Virginia is seeking a Case Manager to support Fairfax County Redevelopment and Housing Authority (FCRHA) participants. You will develop care plans, coordinate housing stability, and connect families with essential resources to promote self-sufficiency.

Responsibilities include monthly engagement, wraparound planning, referrals, documentation, and collaboration with partners to meet grant objectives. Travel within local area may be required; flexible hours may be needed.

Qualifications

• Bachelor's degree or higher in social work, psychology, or human services is required.
• Master's degree is preferred.
• 2-5 years of experience with vulnerable populations is preferred.
• 2-5 years of case management and care coordination experience is required.
• Familiarity with Fairfax County housing subsidies and related programs is preferred.

Responsibilities

• Complete assessments and develop initial ITSPs based on participant needs and goals.
• Provide participant-centered case management with regular engagement and progress reviews.
• Develop and update person-centered plans reflecting changing needs and employment/housing goals.
• Coordinate housing stabilization efforts with partners to ensure wraparound support.
• Facilitate referrals and maintain accurate documentation for compliance and reporting.
• Attend team meetings and collaborate with ServiceSource and affiliated organizations.
